1. Who we are and what this policy covers

NextGen Spine Center (“we,” “us”) is a minimally invasive spine care practice in Trinity, Florida, led by Craig R. Wolff, MD. This policy explains how we collect, use and share information when you visit nextgenspinecenter.com, respond to our online ads, fill out one of our forms, or contact us by phone, text or email.

This policy does not replace our HIPAA Notice of Privacy Practices. Once you become a patient, your health information is protected health information (PHI) and is handled under HIPAA and our Notice of Privacy Practices. This policy covers information you share with us before you become a patient, such as a website or ad form inquiry.

2. Information we collect

Information you give us. When you fill out a form on our website or a lead form inside Facebook or Instagram, we collect what you enter. This typically includes:

  • Contact details: name, email address, phone number, ZIP or postal code
  • Answers about your condition: where your pain is, how long you have had it, treatments you have tried, whether you have an MRI, whether surgery has been recommended
  • Practical details: your insurance or how you plan to pay, whether you can travel to our clinic, and when you would want treatment
  • Anything else you choose to write in a comment box

Information from Meta (Facebook and Instagram). If you submit a lead form in a Facebook or Instagram ad, Meta sends us your answers along with details about the ad you responded to, such as the campaign and ad name. Meta may pre-fill some fields from your Meta profile; you can edit them before submitting.

Information from calls, texts and emails. If you call, text or email us, we keep a record of the conversation, including notes our patient coordinators make.

Information collected automatically. When you visit our website, we and our partners use cookies, pixels and similar tools to collect your IP address, browser and device type, pages viewed, and the ad or link that brought you to us. See Section 5.

3. How we use your information

We use your information to:

  • Have a patient coordinator contact you about your inquiry
  • Review whether you may be a candidate for our procedures and schedule a consultation
  • Verify insurance or discuss self-pay options when you ask us to
  • Send appointment reminders and follow-up messages you have agreed to receive
  • Measure which ads and pages bring in inquiries, and improve our advertising (see Section 5)
  • Operate, secure and improve our website
  • Comply with the law and protect our rights

We do not sell your personal information.

4. How we share your information

We share information only with service providers who help us run the practice, and only for the purposes in Section 3. They are not permitted to use it for their own purposes, except as described for advertising platforms in Section 5.

We may also disclose information if required by law, to protect someone’s safety, or as part of a merger or sale of the practice. If that happens, this policy continues to apply to information collected under it.

5. Cookies, pixels and advertising

We advertise on Facebook, Instagram and Google, and use Meta’s and Google’s tools to measure whether our ads work.

  • Meta Pixel. A small piece of code on our website tells Meta when someone who saw our ads visits our site or submits a form. Meta may use this, together with information it already has, to show our ads to people likely to be interested and to report results to us.
  • Conversions API. When an inquiry moves forward with us (for example, from new inquiry to scheduled consultation), we may send Meta a limited update linking that progress to the original ad response. We send identifiers Meta uses to match the update, such as an encoded (hashed) email or phone number and a lead ID, along with the name of the step your inquiry has reached in our intake process. We do not send your form answers, diagnoses, imaging results or medical records.
  • Google tag (Google Ads and Google Analytics). Code from Google on our website records visits, the pages you view and actions such as submitting a form. Google uses this to measure our ads and give us traffic reports. We do not currently use Google remarketing (showing our ads to people because they visited our site); if that changes, we will update this policy. Google may combine the data its tag collects with information from your Google account if you are signed in.
  • Website cookies. Our site may also use cookies for basic functions and analytics. You can block or delete cookies in your browser settings.

Meta and Google handle information they receive under their own policies: Meta Privacy Policy and Google Privacy Policy, including how Google uses information from sites that use its services. You can limit ad personalization in your Facebook or Instagram Ad Preferences and in Google’s My Ad Center, opt out of Google Analytics with the Google Analytics opt-out add-on, and opt out of interest-based ads from many companies at optout.aboutads.info.

6. Text messages and phone calls

By giving us your phone number on a form, you agree that NextGen Spine Center may call or text you about your inquiry, appointments and related follow-up. Consent is not a condition of receiving care.

  • Message frequency varies. Message and data rates may apply.
  • Reply STOP to any text to stop receiving texts, or HELP for help.
  • To stop calls, tell the coordinator or contact us using Section 8.

We do not share your phone number with third parties for their own marketing.

7. Your choices and rights

You can contact us (Section 8) to:

  • Ask what personal information we hold about you from an inquiry
  • Correct information that is wrong
  • Ask us to delete your inquiry information, unless we must keep it by law or you have become a patient
  • Stop calls, texts or marketing emails

We will respond within 30 days. We may need to verify your identity first. Patients’ rights over their medical records are described in our Notice of Privacy Practices.

Visitors from Canada. If you contact us from Canada, your information is sent to and stored in the United States, where it may be accessible to U.S. authorities under U.S. law. You may ask to access or correct your information, or withdraw consent to further contact, using the details in Section 8.

8. Security, retention, children, changes and contact

Security. We use reasonable administrative, technical and physical safeguards, including access controls on our CRM and phone systems. No system is perfectly secure.

Retention. We keep inquiry information for up to 5 years after our last contact with you, unless you become a patient (then medical-record rules apply) or ask us to delete it sooner.

Children. Our website and ads are not directed to anyone under 18, and we do not knowingly collect their information through them.

Changes. We may update this policy. The date at the top shows when it last changed; significant changes will be noted on this page.
